Form must be completed by all Juniors and Seniors who qualify and are requesting a Home period off. Students just need to complete and submit the second page.
Once you have completed the form, save as a PDF and send as an attachment to your high school counselor
Once you have completed the form, hit submit and it will go directly to the Yearbook Teacher.
ASB Application

Make a copy of the Google Form and type in responses. Once you have completed the form, save as a PDF and send as an attachment to Mr. Selby at fselby@sandi.net
Once you have completed the form, hit submit and it will go directly back to the ASB Advisor. The application is due by TBD.
